McKenzie Health System will be at the middle school media center to host our annual Sports Physicals. This will be for students in grades 5th through 11th. Cost is $15, please have checks made out to McKenzie Health System or you can pay with cash as well. Physical forms are available in the school offices, in the Athletic Office or on the MHSAA website under forms. Physicals must be completely filled out and signed by parent in order to have the physical completed.
